Preparing for 2019-2020: Forms, Downloads, & More

This page includes information to help in preparing for the upcoming 2019-2020 academic year including information about forms, helpful downloads, and FAQs. 

All families must complete ALL forms for their Millbrook student by August 15, 2019.

ONLINE SUMMER FORMS

All permissions and contact information updates are being done through the secure portion of this website, a single-sign-on process. Please note these steps to complete the summer forms:
  1. Please login into the Millbrook website via the Login link at the top of the webpage. Click on the forms link in BLUE at the top of your landing page.
  2. All forms that need to be completed will be displayed. If you have more than one child at Millbrook, you will need to complete a form for each child.
  3. There are forms that need to be signed by at least one parent and the student including the Computer Use and Library Use forms. Although two parent signature boxes might be displayed, only one parent must sign. The student must log in with his/her own username and password in order to sign these forms and complete the submission process.
  4. In addition, all parents should review the contact information we have on file. It is important that we have correct address, phone, and email information for each parent. When logged in, click on the down arrow next to your name in the top navigation bar, then click "Profile". Please review your profile and make any changes, if necessary, by clicking on the pencil icon next to the information you need to edit.
  5. Parents should also download important information on the right side of this screen. This includes general lists of what to bring for boarding students, transportation information, and more. All of these downloads will be available here and on your Parent Resource Board throughout the year.

Magnus Health Forms

Click here to sign in to Millbrook's website - there is a single sign-on that allows you access both to our Parent Resource Board and to the Magnus Health Portal. From the Parent Resource Board, click the Magnus Health Portal button to provide and update all health and immunization records.

  • Where can I find information about summer reading?

  • How do I retrieve my belongings if I used the moving service that is partnered with Millbrook School last year (applies to returning students only)?

    If you used the moving service that is partnered with Millbrook School, your belongings will be waiting for you at your dorm building upon your arrival to campus. 
  • When should I arrive to campus for registration?

    RETURNING STUDENT LEADERS- Tuesday, September 3rd
    Day students expected to stay - free overnight
    10 a.m. – 11 a.m.
    Dorm Leaders, Peer Counselors, Zoo Curators, Head Farmers, and Prefects arrive and register (Mills Athletic Center)
    10 a.m. – 10 p.m.Various student leadership training activities 

    Please note that students leaders will also participate in training activities throughout the day on Wednesday, September 4th.

  • Are there general packing and dress code guidelines that I should be aware of?

    Yes, please see the guidelines below. The following electrical appliances are not permitted in student dorm rooms:
    • refrigerators
    • toasters
    • hot plates
    • heating coils
    • hot-pots
    • air conditioners
    • sun lamps
    • televisions
    • electric blankets
    • microwave ovens
    • space heaters
    • gaming systems
    Lamps, stereos, clocks, radios, computers, fans, and hair dryers are permitted.
     
    Dress Code

    Class DressFORMAL DRESS
    Class dress requirements plus the additional requirements below
    Pants: chinos or corduroys
     
    Shorts: no shorter than 5" above the knee, only allowed to be worn before Thanksgiving break and after spring break
     
    Belts: should be worn with shirts with a classic hem (where the tails are longer than the sides of the shirt), belts need not be worn with shirts that have a straight hem (where the hem is even all the way around)
     
    Shirts: collared shirts, turtlenecks*
     
    Skirts: no shorter than 5" above the knee
     
    Dresses: no shorter than 5" above the knee, straps at least the width of two fingers
     
    Shoes: should be functional for the academic day
     
    Shirts: should have a collar, turtle neck, or be a blouse
     
    Blazers: should be worn with all shirts
     
    Ties: should be worn with any shirt having a collar and classic hem
     
    Shorts: should be worn no more than 5" above the knee, only allowed to be worn before Thanksgiving break and after spring break
     
    Skirts: no shorter than 5" above the knee
     
    Dresses: no more than 5" above the knee and should have straps at least the width of two fingers
     
    Belts: should be worn with all shirts, no matter the hem, including turtlenecks
     
    Shoes: should be formal, sneakers or athletic shoes should not be worn
     
    *between Thanksgiving vacation and March break, long-sleeved crewneck sweaters of a solid color or simple pattern (without writing or graphics) may be worn in lieu of a collared shirt or a turtleneck. These sweaters should not be made of sweatshirt material.
     
    General Rules:
    • Shorts and open-toed shoes should only be worn before Thanksgiving vacation and after March break.
    • Students should not have facial hair.
    • Any non-religious headwear should be removed while inside.
    • Denim should not be worn (denim jackets are permitted).
    • Hooded sweatshirts should not be worn (zippered hoodies and coats with hoods are permitted).
    • Earphones should not be worn during the academic day without the permission of a teacher. Earphones should never be worn outside during the academic day.
    • All articles of clothing should be in good repair, without excessive rips or profane/disrespectful graphics.
    Class dress extends throughout the duration of the academic day. On half-days students are expected to stay in class dress through lunch.
     
    Formal dress extends throughout the duration of formal events on campus, the most common of which are Thursday evenings (Chapel Talks and Formal Dinner). Other formal events might include Convocation or Commencement; students will always be notified prior to the event if they are expected to wear formal dress.
  • How do/when can I ship items to Millbrook School for the upcoming year?

    Packages may be delivered to the school for the upcoming year throughout the last week in August. They should be addressed to the student (please see example below):

  • How does laundry work?

    The school has washers and dryers available for student use. These heavy-duty machines can handle sheets and general laundry. In addition, the school has made arrangements with E & R Laundry to provide a personal laundry service for students. E& R laundry will send a flyer out to all boarding students in the summer regarding sign up. 
  • When do I find out my dorm assignment?

    You will find out your dorm assignment when you arrive on campus for Opening Days. 
  • Where can I find information about ordering course books and the online book store?

  • How do I access the Magnus Health forms?

    Magnus Health can be accessed via the Parent Resource Board on the Millbrook School website. After logging into the Millbrook website via the "Login" link at the top right portion of the Millbrook homepage, you will be directed to the Parent Resource Board and see an icon that says "Magnus Health Portal."  Click on that icon, and you will be taken directly to Magnus Health. No further username or password is required.

  • Do you store Magnus Health documents in hard copy anywhere at Millbrook School?

    No, we do not store these forms and documents in the health center or anywhere at else at Millbrook School. They are found only on the secure Magnus Health portal.
